Essential Documents for Your Pet’s Care

To ensure the safety, well-being, and proper care of your beloved pets, Pet Sitting Services requires specific forms to be completed and signed before each scheduled visit. These documents help them gather important details about your pet, outline their services, and clearly define fees and policies.

Why Are Pet Sitting Forms Important?

  • Liability Protection – Safeguard both the pet owner and pet sitter with legal agreements.

Required Pet Sitting Forms

  1. Dog, Cat, or Alternative Pet Information Sheet
    Share important details like feeding schedules, medical needs, and emergency contacts.
  2. Veterinary Release Form
    Authorizes us to seek veterinary care in case of an emergency.
  3. Client Service Agreement (Contract)
    Outlines our terms, responsibilities, and the services provided.

The Client Service Agreement is not available for direct download.

Upon confirming your reservation request, you’ll receive an electronic Client Service Agreement for your signature. Once completed, we’ll confirm your reservation and send the service invoice.


Booking & Reservation Process

  • Reservations are first come, first served—secure your spot early!
  • After your consultation, most of the pet sitting services will give you a folder containing your Pet Information and Veterinary Release forms.
  • That folder will be used for future visits, and you should simply leave the folder out along with any additional notes.
  • Another consultation is not required unless requested.
